ADVANCE DIRECTIVES/ LIVING WILLS INFORMATION

Once you have completed your Advance Directive or Living Will with Legal, bring a hard copy to the 7th Medical Group Outpatient Records to have it placed in your records.

We do not receive any copy automatically from legal. It needs to be brought in to be added to your records.

Dyess Air Force Legal Office Information

Dyess Air Force Base > Units > Support Agencies > Legal (JA) (af.mil) 

Dyess Air Force Base legal attorneys can draft wills, healthcare powers of attorney, and living wills (i.e. Medical Directives). 

Wills are by appointment only and are in 90-minute blocks. 

You must have a ticket number from the following site before making your appointment:  U.S. Air Force Legal Assistance (AFLASS) 

Once you have a ticket number, please call the Dyess Air Force Base Legal Office at (325) 696-2033 to schedule an appointment.
